If Bolton's man had cut off Jaime Lannister's hand, Bolton would never have let Jaime go. Adaptation always requires cutting and streamlining. As you note in your first paragraph it would be very difficult to include the Brave Companions and the show-only sequence introduces an important character in Locke. I'm not sure it's a plot hole.

Jaime looked nervous for a split second and Ned looked determined . . . If the fight went on a little longer, Jaime would have went for the killing blow in a theatrical fashion to prove how much superior he is to Ned in front of his men . . . Ned would have dodged/reversed it on him and beheaded him in a single blow.
